Help to recover and stay independent (reablement)

Reablement services help people recover, regain independence and remain at home.

The reablement service provides short-term support to help people recover, increase independence and regain skills, safety and confidence.

Reablement may:

  • help you after an injury, an acute illness or an operation
  • help you return home more quickly after a stay in hospital
  • help you remain at home when things become more difficult
  • help you avoid hospital admission

Our goal is for people to regain their independence so that they can stay independent in their own home with the minimum level of support.
 

Getting support from the reablement service

You cannot refer yourself to the reablement service. You may be offered a period of reablement and we will agree a support plan with you.

Support is provided by a team of people. The team will discuss and identify realistic aims for you to work towards during your time with the service. We will set and review goals to help show the progress you have made.

Following reablement, if you need long-term support, we will arrange for a care and support needs assessment to work out how your needs will be met. The reablement care short term service is free of charge for the agreed period. If ongoing care is required, the service will be chargeable.

 


The West Berkshire reablement service is regulated by the Care Quality Commission (CQC).
