This form is for residents to request support with an issue related to the coronavirus pandemic. If you're concerned about a medical condition or are feeling unwell then please visit the NHS Coronavirus Covid 19 website
for further information and advice. If you consider the situation serious please phone the NHS on 111.
If you consider yourself to be a clinically extremely vulnerable person and need support from the NHS you can register yourself, or on behalf of someone else here
If you have been told by the NHS Test and Trace Service to self-isolate, either because you have tested positive for coronavirus COVID-19 or you have been in contact with someone who has tested positive, you may be entitled to some financial support during your self-isolation period. Please use the Apply for a payment of £500 to help you self-isolate
COVID Winter Grant - If you are in financial hardship due to the pandemic, you may be eligible for support for food, utility bills (heating, cooking, lighting), water bills for household purposes (including drinking, washing, cooking, central heating, sewerage and sanitary purposes), or other related essentials. Please use the Apply for a COVID Winter Grant
